What is patient acquisition cost?

Patient acquisition cost is the total marketing and sales spend needed to bring in one new patient who actually attends, divided across the patients acquired in a period. For Indian hospitals it should include media, agency fees, contact centre and referral costs, and be tracked by specialty, because acquiring a cardiac surgery patient and an OPD patient cost very different amounts.

Why it matters for hospitals

It connects marketing spend to the patient who walks in, not the enquiry. It lets leadership compare paid media, doctor referrals, corporate tie-ups and camps on one basis. Read against patient lifetime value and case margins, it tells you how much you can afford to spend to win a patient in each service line.

How to put it into practice

  • Agree which costs are in scope: media, agency retainers, creative, call centre seats, camp expenses and referral payouts where lawful and disclosed.
  • Count only new, unique patients who attended, matched from your HIS rather than from ad platforms.
  • Calculate it per specialty and per channel each quarter.
  • Compare it with contribution margin per patient and expected repeat visits before raising budgets.
  • Recalculate after major changes such as new doctors, new packages or a new branch.

The common mistake

Using media spend alone and leaving out people and agency costs, which makes digital channels look cheaper than they are.

An illustrative example

A hospital group found its IVF acquisition cost was within plan on paper, but adding counsellor salaries and agency fees raised it by a large margin. It then shifted effort towards follow-up of existing enquiries before adding media. (Composite example, not a specific hospital.)
