What is cost per lead (CPL)?
Cost per lead (CPL) is the marketing spend divided by the number of enquiries a campaign generates, such as calls, forms or WhatsApp messages. For Indian hospitals it is the most quoted paid media number, but it only measures the cost of an enquiry, not the cost of a patient who actually arrives and is treated.
Why it matters for hospitals
Agencies and dashboards report CPL because it is fast and easy to move. A falling CPL can hide a fall in lead quality, with more wrong-specialty calls, out-of-catchment enquiries and duplicates. Hospitals that judge campaigns on CPL alone often shift budget towards the channels that send the least useful patients.
How to put it into practice
- Define a lead once, in writing: a unique, contactable person asking about a service you offer, deduplicated across channels.
- Report CPL by specialty and by campaign, never as one blended number across cardiology, IVF and general OPD.
- Place CPL next to enquiry to appointment rate and cost per honoured appointment in every weekly review.
- Set CPL ranges per service line from your own history rather than borrowed benchmarks.
- Audit a sample of leads each month by listening to calls and reading forms to check quality.
The common mistake
Cutting the campaign with the highest CPL without checking how many of its leads became treated patients. A high-CPL campaign that books surgeries is often the best money in the plan.
An illustrative example
A multispecialty hospital saw orthopaedic CPL double after it narrowed keywords to knee replacement terms. Enquiry volume fell, but booked surgical consultations rose, so cost per treated patient improved. (Composite example, not a specific hospital.)
