What is engagement rate in GA4?

Engagement rate in Google Analytics 4 is the share of sessions counted as engaged: by default those lasting more than 10 seconds, viewing two or more pages, or triggering a key event. For hospital websites it is a quick signal of whether visitors from a channel found something relevant, but it says little about whether they booked.

Why it matters for hospitals

It replaced bounce rate as the main quality signal in GA4, so many agency reports now quote it. It helps spot channels or pages that send uninterested visitors. It cannot show appointments, so it must sit beneath enquiry and booking metrics, not above them.

How to put it into practice

  • Mark booking, call click and WhatsApp click as key events so they count towards engagement.
  • Compare engagement rate by source and landing page rather than as a site-wide average.
  • Investigate pages with high traffic and low engagement for speed, relevance and mobile layout.
  • Treat sudden jumps with suspicion and check for spam or tracking changes.
  • Report it as a diagnostic, with enquiries and appointments as the headline numbers.

The common mistake

Celebrating a high engagement rate on blog pages that rank for broad health queries but never lead to enquiries.

An illustrative example

A hospital’s health blog had a strong engagement rate but almost no bookings. Adding clear links to relevant doctor pages and booking options from each article raised enquiries without hurting engagement. (Composite example, not a specific hospital.)
