August AI turned 9 million free chats into a $39 care plan

August AI, whose free health chatbot has more than 9 million users in 160 countries, has launched a $39 a month US care membership, Fierce Healthcare reported on 23 September. It is the clearest example yet of a free AI conversation used as the top of a paid care funnel.

What happened

Fierce Healthcare reported on 23 September 2026 that August AI, founded in 2022 by CEO Anuruddh Mishra, has come out of stealth with a direct-to-consumer telehealth service in the US. Its free AI chatbot works in more than 30 languages across 160 countries and has over 9 million users.

The paid tier, August Care, costs $39 a month. It includes unlimited AI health chat, one telehealth visit a month (extra visits cost $19), next-day labs, prescriptions after physician approval and a year of follow-up messaging. A licensed physician oversees prescriptions and clinical decisions. The company says it has treated 100,000 US patients, has raised $3 million, is backed by Accel and projects savings of up to $3,000 a year for a typical member. Mishra said it will focus first on uninsured and underinsured people, with insurance reimbursement on the roadmap.

My take

Look at the funnel. Free, unlimited chat in the patient’s own language builds the habit. The chat learns what worries you. When you need a prescription, a lab or a doctor, the paid plan is one tap away. That is patient acquisition done by a product, not by an ad budget.

In India the same behaviour is forming without the membership. Patients already ask ChatGPT, Gemini and symptom apps before they call a hospital. Whoever answers that first question shapes which specialty they think they need, and often which hospital they trust. Hospitals are mostly absent from that moment. Their chatbots book appointments and deflect.

I am not suggesting every hospital should build a consumer AI. Most would do it badly. But you do need a view on where your patients now start, and how your content, doctors and booking flow show up when an AI is doing the answering.

What most coverage missed

$3 million is a tiny raise for 9 million users. It suggests that acquiring a curious user through AI chat now costs very little, while acquiring a patient through search ads keeps getting more expensive. That gap will pull more players into free AI front doors.

The other point is trust. The company cites a 100% score on the US medical licensing exam, but exam scores are not outcomes. The first high-profile error at this scale will shape how every AI front door is regulated, hospital ones included.

What I would do

  • Audit what the major AI assistants say about your hospital, your doctors and your top procedures, and fix the content they draw on.
  • Measure your own chatbot on resolution, not containment: did the patient get what they came for?
  • Design one subscribed relationship, such as a chronic care or annual health plan, that gives patients a reason to stay with you between visits.

  • Whether August takes its paid care model outside the US.
  • How regulators treat free AI chat that feeds a paid clinical service.
